Maternal and Child Health Home Visiting:

Prenatal education and counseling to high risk, low
income pregnant women in order to prevent complications
with pregnancy and improve birth outcomes. |

Women, Infants, and Children (WIC): |
Supplemental food program for eligible pregnant, breastfeeding women;
infants and children under age 5 with a strong nutritional and educational
component. |

Healthy Beginnings: |
Postpartum home visit offer to all mothers and their newborns to provide health education, screening,
resource and referral. |

Follow Along - Early
Intervention
Program: |
Computerized tracking
system to help identify delays in infant/child growth and development through
written questionnaires that are mailed out to families. |

Child and Teen Checkups outreach: |
The goal of
this program is to improve the health of eligible
children and reduce the negative impact of health
problems by advocating regular and comprehensive well
child check ups. |

School Health: |
Mower County Public Health offers comprehensive school health services to select
schools in Mower County. |
|
Child Passenger Safety Seat Program:
|
Public Health Nurses certified by the National Highway Traffic Safety
Association provide education and training with return demonstration, to properly install
car seats. See calendar of events for schedule of clinics. |

Early
Hearing Detection and Intervention: |
Public Health Nurses work with the Minnesota Department
of Health to follow up with families whose children need
further testing for hearing. |

Family Planning
Services |
|
|
Open Door Clinic: |
Provides education, counseling and medical family
planning services for at risk, low income men and women
with the purpose of promoting health and preventing
unintended pregnancies. Clinics are held the 1st
and 3rd Tuesday of each month from 5 pm - 9 pm by
appointment only. Call (507) 437-9770 during
regular business hours to schedule an appointment. |

Minnesota Family Planning Project: |
The Minnesota Family Planning Program is a health care
program that covers family planning services and relates

Disease Prevention and
Control Services: |

|
Minnesota Vaccine for Children: |
The Minnesota Vaccines for Children Program (MnVFC) is
an enhanced version of the federally funded Vaccine for
Children (VFC) program with the goal of ensuring
affordable vaccine for all children in the state. |
|
Immunization Clinics: |
Pubic immunization clinics are held every Friday from
3:15 - 4:30 pm. There is a fee of $14 per dose.
No appointment necessary. |
|
Immunization Practices Improvement Program (IPI): |
IPI addresses several aspects of the immunization
program: vaccine management, vaccine accountability and
clinical immunization practices all help to provide
quality assurance. A public health nurse is
trained to help local vaccine clinics with their vaccine
programs. |

Lead Program: |
Mower County Public Health offers education, counseling,
information and referral to reduce or prevent exposure
to sources of lead, especially in young children. |
|
Blood-Borne Pathogen Program: |
Mower County Public Health provides presentations for
groups on OSHA's Blood-borne Pathogen Standards. |
|
Perinatal Hepatitis B Surveillance: |
Mower County Public Health works with the Minnesota
Department of Health to identify and provide follow up
to mothers who have tested positive for Hepatitis B.
Follow up continues after the baby is born to assure
that the baby has been properly immunized against
Hepatitis B and the proper immunological testing is
completed. |

Freedom to Breath: |
Smoking is prohibited in indoor workplaces and indoor
public areas. Public Health serves as a place of
notification for the general public in the event they
are concerned regarding noncompliance of the law. |

Statewide Health Improvement Project: |
SHIP interventions focus on policy, systems and
environmental changes designed to help all of us make
healthier choices where we live, work and play. |

Blood Pressure Clinics: |
Free blood pressure clinics are held at numerous sites
throughout the county. |

Adult
Health Services:

|
Minnesota Senior Health Options (MSHO): |
Care coordination for persons over age 65 on Medicare
and Medicaid in Mower County. |
|
Personal Care Attendant Screenings: |
Trained Public Health Nurses assess and determine an
individual with disabilities needs in accomplishing
activities of daily living and health-related tasks so
they may continue to live in the community. |

Disease Prevention and

|
|
Refugee Health: |
Public Health Nurses assist new refugees in our
community to access health and information services as
well as complete Tuberculosis screening. |

Communicable Disease Outbreak Investigation and
Follow Up: |
Investigate outbreaks of communicable disease in
cooperation with the Minnesota Department of Health. |
|
Public Health Preparedness: |
Coordination of local, state and national planning and
preparation to act in the event of a bioterrorism or
other public health threat. |
|
Tuberculosis Case Management: |
If a person is diagnosed with latent or active TB a
public health nurse will case manage the case by doing
further contact investigation and medication management. |
